Insurance Update: As of January 1st, 2026, Partnership Community Health Center Dental will not accept commercial dental insurance plans purchased individually or provided by an employer. If you have BadgerCare Plus/Medicaid (i.e. the ForwardHealth card) in addition to a commercial dental plan or Medicare, we welcome you to receive dental care with Partnership. We also welcome patients who only have BadgerCare Plus/Medicaid as insurance or those who do not have dental insurance and qualify for our sliding fee scale.
Cancellation Policy
We require a minimum of 4 hours’ notice when cancelling an appointment. This makes it possible for another patient to be seen at that scheduled time. If you are unable to give us at least 4 hours’ notice, it will be counted as a “missed appointment” and could affect your future scheduling privileges.
