Our mission at Partnership Community Health Center Inc. (PCHC), is

Partnership Community Health Center (PCHC) has been providing quality health care to our community for over 25 years.

Founded in 1997 as a free clinic called Fox Cities Community Health Center, we went on to become a federally qualified health center (FQHC) in 2004 and, in 2013, we changed our name to Partnership Community Health Center. PCHC now cares for 16,000 patients annually and employs nearly 120 staff from the local community. Our care model includes primary medical, dental, integrated behavioral health and comprehensive outreach and enrollment services. 

We respond directly to the ever-changing and growing needs of our community. By focusing on patient-centered care and outreach and enrollment approaches, we connect those in need to care and strive to create a healthier and more vibrant community.
