Integrated Behavioral Health

As a part of our “whole person” care approach, Partnership integrates behavioral health with a patient’s medical care, providing access to mental health services right when they are needed. Our primary care physicians and behavioral health consultants (BHCs) provide integrated care that offers an efficient way to address the greater need for behavioral health services among our patients.

Our BHCs assist patients when habits, behaviors, stress, worry or emotional concerns interfere with daily life and overall health. Concerns can be about physical problems or other life problems.

Together, the consultant and your primary care provider collaborate to help you to improve your overall health and well-being.

The Behavioral Health Consultant will meet with you in your primary care provider’s exam room. The visit will be brief (20 – 30 minutes) and focused on clarifying problems and agreeing on solutions. This service is another part of your overall health care and is not therapy.

Here are just a few examples of how our BHCs can help:

  • Depression
  • Anxiety
  • Managing Life Stress
  • Loss or Grief
  • Substance Use
  • Relationship Support
  • Anger Management
  • Health Goals and Behavior Change (e.g. Diabetes or high blood pressure management, smoking cessation, goal setting and tracking)

Ask to meet with one of our BHCs at your next primary care appointment!
