The mission of Partnership Community Health Center, Inc. (PCHC) is to increase access to great care. We welcome you to schedule an appointment with us! Because we are a Federally Qualified Health Center, no patient needing services will be turned away–we have reduced fees and flexible payment plans based on your ability to pay.
Scheduling an Appointment
To schedule an appointment, please call (920) 731-7445.
Make sure you come to the clinic prepared for your appointment. We accept Medicaid, Medicare, and other insurance providers. If applicable, please bring your insurance card. If you are new to the clinic, a pre-registration visit prior to your medical appointment is encouraged.
Discounts are available to residents without insurance; there is a co-pay expectation for each service. Please bring the following when visiting the health or dental center:
- proof of family income
- photo identification
- social security number
- If you are unemployed, a letter is required from whomever is providing shelter and support.
Co-pays are due at the time of service. To estimate your co-pay based on your income and family size, please see the fee chart here. Our staff will also be able to tell you what your co-pay is when you visit the health center. If you do not have insurance, we offer payment plans that are based on your ability to pay.

Late or Missed Appointments Policy
We realize that life can often complicate the best laid plans, however, if you are going to miss an appointment, cancellations are graciously accepted 24 hours in advance. Excessive late arrivals or appointment no shows will result in termination from the clinic.
Our clinic is very busy—please help us help as many people as possible by showing to your appointments on time.
